Services:

• Provide free preventive health assessments which include weight, height, blood pressure, pulse, oral exam, foot exam, vision check, hearing assessment, review of diet, review of medications, and review of health history.
• Provide counseling and instruction on health promotion activities, self care and risk reduction.
• Assist adults in controlling their health.
• Group health education in the community.
• Community leadership and advocacy regarding senior issues.

Target Population: The non-frail, community dewlling elderly older adult age 50 and above who is at risk and/or under-served.
