Welcome to the Stanislaus County Health Services Agency Mobilizing for Action through Planning and Partnership (MAPP) website.

We hope that this will allow you easy access to essential information pertaining to the MAPP process as it is currently being implemented in Stanislaus County.



To sum it up in a few words, Mapp is looking at ways of improving health at all levels, including:

  • Community level (community residents & community based organizations)
  • Local government level
  • State level

TIMELINE OF MAPP ACTIVITIES


  • Completed -- Analysis of data collected for Phase II of the Community Health Assessment (Community Health Status Assessment). Phase I of the Community Health Status Assessment is complete. This information is available by visiting the HSA website.


  • Completed -- Consolidation of reports related to the Community Themes and Strengths Assessment


  • Completed -- Consolidation of reports related t the Force of Change Assessment


  • Completed -- After approval by Senior Management the Local Public Health System Assessment will begin. This will include a three different group meeting a various points during a day scheduled to complete an assessment tool developed by CDC and NACCHO with members of the local public healthy system.


  • In Progress -- Desired completion of the MAPP process and implementation of the Action Cycle.
