process as it is currently being implemented
in Stanislaus County. |
To sum it up in a few words,
Mapp is looking at ways of improving health
at all levels, including:
- Community level (community residents &
community based organizations)
- Local government level
- State level

TIMELINE

of data collected for Phase II of the Community
Health Assessment (Community Health Status
Assessment). Phase I of the Community Health
Status Assessment is complete. This information
is available by visiting the HSA website.
- Completed -- Consolidation
of reports related to the Community Themes
and Strengths Assessment
- Completed -- Consolidation
of reports related t the Force of Change Assessment
- Completed -- After
approval by Senior Management the Local Public
Health System Assessment will begin. This
will include a three different group meeting
a various points during a day scheduled to
complete an assessment tool developed by CDC
and NACCHO with members of the local public
healthy system.
